Sheesh, looks like the tier 3 ( Level 50 ) Unforgiving Dead gauntlet needs some more work done on it. We made it to the last boss, only she wasn't there! She never spawned! Approaching the area where she's supposed to appear would initiate a battle, but because there were no enemies, it would end almost immediately.
