it used to be true that when you polymorphed, your name was changed, but it was not permanent. I do believe KI has fixed this issue.

With the current moon spells your name is uneffected....you remain the same name.

it used to sometimes happen, but not pernament, but they patched the bug

No! That is false!

Sometimes, when you use a polymorph, your name changes for a few minutes afterward, but I believe that was fixed. So your name doesn't change at all anymore.
