Well i am lvl 46 and to tell ya I don't even have one so I guess they are pretty rare. :)

Death cyclops are obtained by General Greystone in The Atheneum; Plaza of Conquest.

They are generally rare, considering not many people have them, but if you mean by drop rate it is random.
