If you do not have a quest selected, the Quest Helper Arrow will not appear.
Pressing T on your keyboard will turn on and off the Quest Helper Arrow.
Try pressing T on your keyboard the next time you notice the arrow has disappeared. If it doesn't return, it could mean that a) you don't have a quest selected or b) the quest you have selected does not qualify for the Quest Helper Arrow
