Quest arrow disappearing randomly

What the title says.

I've been questing on my fire, and after I exit a boss' lair or area, the quest arrow often disappears. This is very annoying, and seems to only work when you sometimes switch between quests, log out then back in, or just entering an area.

Is it disappearing predictably when a certain quest is finished or begun, or is it random for the same activity?

That happens to me a lot as well and it happens when new quests are received usually, regardless of which quest it is.
