Competed my original Empyrea Storm Titan Maelstrom quest via Team-Up. When I attempted to continue to Aethyr Titan (which does not appear to be a separate instance and has no Team-Up option or sigils) from the Storm Titan dungeon, I was kicked back to the Team-Up kiosk with no dungeon recall. When I returned to Husk, the Storm Titan was again alive and well. Any assistance would be greatly appreciated.

I had the exact same issue after defeating Titan and exactly the situation you describe. I suggest you contact Customer Support with exact details of what took place. They were of great assistance in helping me resolve.
