The stone rose in the Grand Chasm is a little tricky. The directions on the link above tell you that The Grand Chasm Stone Rose is in The Grand Chasm Past instance. As soon as you enter, turn right and go all the way to the end of the hall. The Stone Rose is on the leftattached to the last vault door (look up).
When you go down the hallway inside the Grand Chasm Past instance, and you look to your left, you almost have to "jump" to get the stone rose. It is on the wall, but very high up on the wall.
